Your First Visit What to Bring What to Wear Initial Evaluation Treatments Your First Visit:
Upon arriving to VTFC for each appointment you will need to sign in at the front desk. It will be necessary to have your new patient paperwork completed either before arriving or at VTFC prior to your initial PT evaluation. You may download your new patient forms from our website. We would like each new patient to arrive to their first physical therapy appointment approximately 10 minutes early if your paperwork is completed, or 30 minutes early if you have not completed your new patient forms. This will enable us to confirm all of your completed paperwork, photocopy your insurance card and identification, and have you change into appropriate clothing for your initial evaluation. VTFC has changing rooms and showers available for the convenience of all VTFC clients. If you by chance forget appropriate clothing for your initial evaluation or treatments we have extra shorts and t-shirts available to be used during your visit. What to Bring All new patients need to bring in the following information for their first appointment: - Completed New Patient Registration Packet (approx 6 pgs)
- Completed Injury Assessment Form (specific to your injury)
- Current Health Insurance Card(s)
- Prescription from referring physician
- Any X-rays, MRIs or physician reports related to your injury
- Please make sure all forms are thoroughly read and completed. - We request you arrive 10mins early prior to your first appointment. - If you have any MRI, X-Rays or physician reports please bring them with you. - Patients are responsible for contacting their insurance company prior to their first appointment to verify their Out of Network Benefits for Physical Therapy. What to Wear: What to wear will be based on the area of your body that is to be evaluated. See below: Low Back – Shorts and T-shirt Neck – Tank top Mid Back – T-shirt and shorts (sports bra for women) Upper Extremity – T-shirt Shoulder – Tank top Lower Extremity – Shorts Foot & Ankle – Shorts, athletic shoes and previous orthotics (if applicable) Initial Evaluation: Your first visit will be your initial evaluation which will be a one-hour visit that will consist of you and your physical therapist discussing the reasons for pursuing physical therapy, as well as special tests to determine range of motion, strength and the integrity of your neurological system. Each initial evaluation will also include specific hands-on tests to determine how your affected joints are moving and whether certain joints are contributing to your current dysfunction. Do not be surprised if during the initial evaluation our physical therapists investigate joints distant from your pain as some pain can be referred due to musculoskeletal compensations, such as low back pain from foot dysfunction. At the end of the initial evaluation your physical therapist will explain the findings and present a treatment plan to improve your condition. Every patient is different so it is extremely important to realize that treatment plans will be unique to the individual and variable from patient to patient. Treatments: Each subsequent treatment to your initial evaluation will most likely be one hour in length. Our goal is to have each patient remain with the physical therapist that performed the initial evaluation so that the treatments reflect a level of consistency for each and every treatment. There are exceptions when some patients will be seen by a different physical therapist, but we do our best to have all of our patients remain with their primary physical therapist.
